Salary:$17.00 per hour

Responsibilities:

Provide personal care under the directsupervision of a licensed nurse. Personal care services mean some or totalassistance with personal hygiene, dressing, and feeding; nutritional andenvironmental support functions; and health-related tasks. Such services mustbe essential to the maintenance of the patient's health and safety in his orher own home based on the care plan prepared by a nurse. Providing personalcare services carries out the functions and tasks identified in the patient'scare plan.

Requirements:

• Pre-Employment Physical
• Rubella/Rubeola Lab Report (with titer numbers)
• Drug Screen (8 Panel lab results required)
• HHA Certificate (Original Copy)
• Two Professional References
• Valid Picture ID
• Social Security Card
• COVID Vaccination Proof
• Chest X-Ray Report (Please provide if PPD is positive)
• Flu shot (Encouraged/Recommended)

Other Requirements

• Must be fully vaccinated and proof of vaccination will be required.
